Say a Pistoleer shoots amob for 800 damage, he gets the hate (meaning the mob attacks him), then a Rifleman shoots the samemob for 900 damage, he now has the hate and gets attack. You heal the rifleman for 1000 damage, you now have the hate and it switches to you.
Think of your heals in terms of damage points and it helps to understand how much hate you will get for healing.
There are many situations where it is better to let someone incap than heal them, you can always drag/heal or drag/rez them afterwards and tell them not to aggro so many mobs at once.

at least they reduced hate from healing by 50% in one of the last patches - so a heal of 1000 pts will generate the same hate as if u hit the mob with 500 damage.
the biggets problem occurs if a player is attacked by more then one mob (social or agro creatures). when he is doing only damage to 1 attacker then even the slightest heal will outdamage him on all the other mobs - bringing the healer on the top spot of the hate list of all the other mobs. thats one of the situations where its better to stop healing and clear the mess afterwards (heal/rez incapped/dead players) - as noone can rez when the doc is the first one who dies.

There's an excellent sticky in this forum titled "Learning to manage hate 101" that you should read. It's highly informative and helpful.
