Add to your list:
Player listening to music while getting a dance buff
Player running out of range during buffing (not exactly sure what the range is)
It may be safe to assume that just about anything a player does that requires a command seems to be interfering with entertainer buffing. It's best to let them know that things are really touchy with buggy buffing code right now, and advise that they just stand and watch until you're done. Then engage them in conversation to pass the time if they're not the sort that flips out having to speak to another person.
And yes, it's normal for the dance buffs to be a few seconds over the 2 hour mark. We just round it off to 2 hours when we talk about it.

I think so... on both sides. I've seen them going wonky if I /invite someone else to team while never changing my target. I stopped inviting while buffing. *shrug*
For the record, I bugged all of this right after our "fix" patch... I've been told they are working on it. I hope so, it is annoying.

All entertainer buffs are whatever and 10 seconds. So the max is 2 hours and 10 seconds. A 15 minute buff is really 15 minutes and 10 seconds. Smallest you can get is 1 minute 10 seconds.

I don't think it's happened to me yet, but I know Smiling (on Scylla) has had trouble with people sitting while he was buffing them.

Well, patrons don't say "YOUR BUFFS ONLY Last 2 SECONDS.. LIEK Aom G I watstse 5kl!!!!!!" They respect Dancers and don't simply tell us when someone goes wrong. Like the actions people have said, they probably cause a error/glitch in the buffing process. I buff friends all the time for 2 hours but when I buff people they get buffed for 15 mins??? LMAO. Im just better off buffing people for 7 mins *because there are some very dumb patrons* so they will come back to me in the future...

Last night my musician/dancer friend had the same problem with buffs lasting too short but only when using /setperform. When in the group there were no problems. Also I need to /watch or /listen two times nowadays instead of one time for a buff to be applied. No idea why.

One problem I've noticed recently is that if I'm getting a muscian buff, I havegotten into thebad habit of doing /stopwatch instead of /stoplisten. I'm so used to telling people to type it, that I end up doing it myself. This seems to cancel out the music buff, and then I have to listen all over again. I wouldn't be suprised if it worked the other way around too.

*perk* Perhaps a clue to the same time Musician/Dancer Buff Bug? I'll try this one and turn it over to QA if it's recreatable.

Good catch Panthu.
A variation to try... Go for both buffs, but wait 2 minutes between stopping watching and stopping listening. Try the other way too, stop listening first then stop watching.
One thing I've found in my testing is that even when a buff fails to apply due to not watching long enough (Possibly other reasons too?) you don't lose the progress. Just start watching again and after 2 minutes you can get the full buff.
