Well, at the very least I think it's safe to say that the accuracy mod is not just a percentage. If it were then a TKM would never miss (not taking into account dodge, block, or counterattack).

The dice analogy is correct. It's about a random number generator. The random number migth be anything form 1-100. Now suppose the following scenario:
Roll needed to hit: 50 or higher
Target's ranged defense: 20
Roll needed to hit THIS target = 50+20 = 70
Now, lets say you roll a 60. Without ANY skillmods, you would miss. But if you have + 25 to pistol accuracy, your roll becomes 60+25=85. And since 85 >70, you score a hit. THis is a very simplifiedexample becasue in actual game, your posture, your motions as well as th target's posture or motion are also figured in. But basically, your targets defense modifiers are subtracted from your roll and your own accuracy modifiers are added to your roll and if the final number is equal to or larger than some arbitrary base number, you score a hit.

I think there are other considerations to take into account besides this. For example I've swung at low level NPCs with no melee defense before as a TKM and on occasion have had a clean miss. According to your formula a target would need a melee defense of +25 or better tobegin to affecta TKMs unarmed accuracy, which is well over 100.

No, not unless said TKM also has Pikeman or Rifleman in his template. Block is the method of defense used by those two professions. While TK has the ability to block, it is granted to us through our Defensive Acuity.
The only tapes that would "help" us would be for Defensive Acuity, which therecurrently aren't any SEAs for.
