Poisons do injury damage (white) which can be healed by anyone using a stimB (except for MIND) and regenerated over time. Diseases do wound damage which can only be healed by a wound pack and medical droid (or MIND wounds needing an entertainer) The wounds being permanent until they are healed.
Anything that is permanent has to be given a lower damage base or you're looking at a major game inbalance no matter what pool you are attacking, hence diseases with effectiveness of 200 and tick every 40sec.
Stacked with a poison the battlefatigueincurred by the disease causes one or the other to go over the 1point cap and incapacitate the victim. Since it is a DoT it causes the triple incap that so many people hate.

A disease ticking on someone that happens to have high amounts of BF can be terribly effective. Turns what might be a 200 tick disease into a 600 - 1000 tick disease.

Hi folks, I'm a new CM and I just had one quick question. I've made some (for me anyhow) goodmind poisons for pvp use, and have been having fun with them (tick around 700). I just crafted my first mind disease C, and compared to my poison it sucks. Does anybody use mind disease? Almost every battle I've been in has ended within 1 min of it starting, so we're only talking 1 tick on the mind.

Yes, people do use diseases in PvP combat. The primary use of it is to take advantage of the fact that diseases (possibly when crossed with poisons, possibly on their own) can ticka person below 1 HAM and incap him or her.
Some secondary uses for diseases are to try to apply lots of wounds to take somebody out of combat for longer than a simple death would. If you can manage to pull it off, getting a rifleman, doctor, combat medic (or pretty much any class) to having a maximum of 50 mind points is a very effective way to nullify a threat for a good5 or 10minutes or so (more if there is no master musician or dancer around). There is also the ever... popular... griefing of people as well.
Finally, they are useful for getting the last word in in combat. If somebody TEF attacks me, my sole goal is to disease them (I am unlikely to be able to beat them) to hopefully make them regret picking on a "harmless combat medic who wasn't bothering anybody" and idealy not bother me in the future... It may not be "useful", but it is a very effective deterrent for non-doctors non-Teras Kasi, assuming you are displaying your master combat medic title... 
However,in a "good, clean PvP fight" (such as casual dueling), and even the majority of medium-large scale PvP battles, diseases are of very limited usefulness as you noted...
