Lets kick this around a little.
Why not keep everything the way it is but give them range mode for accuracy. I mean why should the poison or disease be apply like medicine w/ almost no chance of missing. Do they ever miss anyways?? Not thatIknow of(I have been wrong before). Make it like ranged combat as the distance increases the chance to miss increases.

We have a "range mod" so to speak. Its called potency. The problem is that most CM stuff made has a potency well over 100 (mine are 120 just from the sub-components). I have yet to have someone resist a C level poison/disease.
Yes we do miss, very rarely. It pretty much has to be a low level poison/disease with a potency less than 100.
