Combat Medic/Master Doctor usually keep their parties healed almost all of the time, and if they aren't watching themselves then you don't have very many opportunities to throw in a whole lot of poisons.
My suggestion, since the MCM/MD combo is kind of a support role, is to take Squad Leader and Combat Medic. Spend the rest of your skill points in a ranged category like carbines or rifles.
A guild leader is a kind of support character in and of itself, so if it's support you're after then go Squad Leader to boost your groups defenses; with Combat Medic it will be pretty easy to keep them healed as long as you get the full mind buffs (music/dancing/food/drink.)
If you want to deal out the damage then use Combat Medic with another crowd control combat profession (either melee or ranged, but you can't master melee.)

I have exactly that template, and I'm a guild leader. The guild members most definitely do not see me as weak. I love this template, but it's not for everyone. If you're the kind of player who enjoys keeping a group on its feet, you'll love it. If your someone who gets caught up in how much damaage you can do, you'll hate it. Your PVP damage will be respectable (though also negatable by a good doc), but in PVE it'll take you forever to kill anything yourself.
