What do the different stats for poisons and disease do? Which are more important? Is the higher the potency the more damage it does? does more effectiveness mean you will hit more often? What other stats are important to consider when making poisons and diseases? Thanks.

Potency affects the chance for the disease or poison to stick while effectivness effects how much damage or wounds it will do. When making the poisons and diseases on the schematic you'll see on the right under the picture what stats on the resources effect what part of the unit.

96 potency seems to be the magic number for me. I hardly ever get resists when i use a 96 pack. even on krayts, spellweavers kimos, etc.

I'm not sure if potency is a direct precentage. In my expirence AoE poisons can be resisted much easier at the same level potency as single target. 85 potency single 90 potency AE seems to do very well for me.
