A 'tick' is when the poison/disease actually effects the opponent.

After application, it takes 10 seconds for a poison to tick, which will then tick every 10 seconds thereafter. After application, it takes 40 seconds for a disease to tick, which will then tick every 40 seconds thereafter(this info is located in the FAQ.)

3) As a CM should a worry about defs and other things?

Depends on how you play, armour used, foods used, wether it's PvE or PvP, etc. But, most likely, yes, you should worry about defs and other things.

4) is doc a good pro to have with CM to giveyour self buffs

Doc is a good complimentary prof to have with CM if you plan on playing the support role. It's the template I have and enjoy.
