From what I have seen, you max out as per your CL just like you would for your weapon exp.
So basically as long as you are healing damage on someone a lot higher than you you should be max exp per combat.

Bigger groups are better when doing missions as there will be more mobs per lair and therefore more damage that needs healing. Last night I was in a full group with 3 80's (me healing and 2 others) the rest being newbs I ground out the wholewarfare line in about 3 hours.

Med xp is just like weapons/combat xp. It's based on the difference between your level and the level of the perps your group is in combat with. If you're fighting level 80+ pikets for example and you're level 80 or lower you're getting the same amout of med xp as you would get shooting with your CDEF pistol one shot/hit. The lower your level relative to the combatants aggroing your group, the higher your xp will be (assuming you land a heal in combat).
You must land one heal, and it must be during combat. Watch the action bar of those you're healing. If it's <100% then they're in combat and you'll get credit.
Hope that helps.

Acutally that's not always true. Some actions are non-combat actions, like CoB, powerboost, etc. So action is not really the best indication of whether someone is in combat or not. Just look at their role icon - the symbol next to their name in the group list - if it's red and flashing they're in combat, and if it's white, they're not. Also keep in mind that even if YOU are in combat, if your healing target is not in combat, you will not earn xp.
