The extra shuttleport on a planet is a bug in the system. One of those cities doesn't have a shuttleport, even though they are still showing on the map. You can still shuttle there, but you'll be standing where the shuttleport used to be with no way to shuttle back out.
seen that happen before and it was the most out of the way city on lok at the time

I have been to all 11 shuttles on this planet and each had an active shuttle in which i could shuttle in (and out of).

There is no waiting queue. Once the system recognizes that a slot has opened (sometimes it takes a while for it to realize this which leads to more than 10 ports), whichever city's update cycle is nearest will get the slot.
