Action Shot 1 will get the bleed 1st hit 99% of the time, unless you miss.

You will see the "blood" indicator by your enemies ham bars, under your current target heading.

Also, you will see the blood drop graphic fade over the toon of your enemy, you will occasionally see a -xxx float above the toon even if you haven't recently struck it.

I don't know why a person wouldn't want to get a bleed on an enemy with a huge ham. Especially an enemy that doesn't death blow. Because if you get incapped, it keeps taking damage

(it will become more evident that the enemy is bleeding if you get incapped and look at your enemies ham.)

ALSO, an enemy USED to bleed out all the way to death, now the ham bar stops at 1. BS.

Also, you know, that Action shots bleed the green bar, the "action" bar...

So, if you're in a group, try and coordinate your specials accordingly.
