Chiming in with some small examples of cover & concealment from the military.

Cover = Shooting from a building window, behind a wall, or firing from a foxhole and using your sandbag/dirt mound as support. Pretty much physical protection from enemy fire, whether that be direct (small arms) or indirect (mortars, howitzers).

Concealment = Camouflaging your gear, position, or yourself to make detection harder. Camouflage face paint, using vegetation, even proper clothing/uniforms for your environment.

Example: You can hide yourself behinda bunch of bushes to provide excellent concealment for laying an ambush. You feel you will be very, very hard to detect by your opponents. However, when the bullets start flying, those bushes you're hiding behind, though they provide excellent concealment, they provide absolutely no physical protection... no cover against enemy fire.
