There are skill tapes for both and my stats show 110 counterattack from heavy swords and 0/+8 further down under two handed. (Using a +8 two handed melee counterattacks skill tape)

A "two-handed melee counterattack" would only work when wielding a two-handed weapon as opposed to the counterattack skill whichwill work when either a two-handed weapon or carbine(?) are wielded. However a lot of skill tapes were left in game even though the skill mod itself was removed and is redundant - thismay be one of those. If it is a working skill mod then theoretically it should stack with your normal counterattack up to the new 125 cap,otherwise +8 modifier is not likely to do very much on its own. I can't say whether this is the caseor not as I am not sure how the rolls are calculated.
The devs said that Dodge, Block, CounterAttack, Ranged/Melee Defense were internally capped at 125. The UI could show a number above 125, but nothing beyond would count.
