It could just be a really bad string of luck. I have had this happen sometimes. I belive there is a random conpondent associated with what happens when you sample.

I was wonder, I finaly find an almost perfect spot, no mobs and a 94% concentration. Oh BTW I have all the survey skill tree. Well my first sampleing was a success and I got 11 units but then 6 times in a row I get a failure. What are the odds of that, it must be a bug of some kind. THey realy need to overhall the mining system.

I must have really bad luck if I am supposed to get a 94% rate off the bat, and then have all of those skills.

Random Number Generators are "sticky" (SOE's more than any other) in that they tend to do lots of repeats of certain values. Hence, when you get 1 failure, you're bound to get a few more within a short time span, then not got any for a while.
This was blatantly obvious on Everquest, where you could be a Grandmaster anything, craft 100 items, then have 3 or 4 fail in a row. Honestly,I think it rewards people who craft or survey a few times here and there, instead of just plopping down for a huge session at once. (Of course, then there's guys like you who just have bad luck).

This sounds like DAoC!! Their idea of random was like this:
You swing, you miss.
You swing, you miss.
You swing, you miss.
You swing, you miss.
You swing, you hit.
You swing, you miss.
You swing, you miss.
You swing, you miss.
You swing, you miss.
You swing, you miss.
You are dead.
This would happen to me once every few days. I would "randomly" miss, and the longest string of random misses was 14 in a row!!

They should enter a pretty "simple" string in the code that says that "miss effect: 10%(or more)less chance to miss on next hit", in this case meaning that if you already had 90% chance to craft an item, you WILL get it crafted on the second try. For newer artisans that would help by not getting 239568 fails in a row, for masters, it means they are almost always certain to "learn from their mistakes" and get the item on the second run.
