The point is that the theater belongs to the Musician or Dancer, not the city.

These can only be placed in citys os sufficiant size, but don't go thinking they're civic structures under the mayor's control. Cantinas, hospitals, and theaters take up the players lots, and belong solely to that player.

I think he means the fact that you can't heal wounds outside in the actual theatre part, nor decorate the stage with anything but lights and other decorations. Essentially it is 3 lots to have a barren stage and 3 slanted concrete slabs while all the nice looking decorations and healing capabilities are inside in that small little backroom.
