I have a question about designating a house, or portion of a house as a cantina or medical center. Is this at all possible? How is this done?

All house currently work as hospitals and cantinas.

You cannot "officially" designate rooms of a house anything. However it will be obvious when you enter a player hospital which room is for healing and which room is for the Doc's crafting.

Player cities ought to be providing us with "real" hospitals and canteenas to build for our towns though.
